Information for New Patients

FAQ's —Frequently Asked Questions

Are you accepting new patients?

Yes! If you are looking to make an appointment simply call us at 732-738-8855.

What should I bring to my appointment?

It is important that you arrive early to your first appointment in our office. You should bring your photo ID, Insurance card, Medication List, and the information from your Primary Care Physician. If your insurance requires a referral, make sure to bring it that day.

I need my records sent to another physician.

We would need to obtain a medical records release on file (either from you or your physician) , as well as the fax number and address of the physician you want us to send your records to.

Can I get a cardiac clearance at this office?

Although our doctors issue cardiac clearance before a procedure, you should make an appointment at least 2-3 weeks before your surgery is scheduled. This will allow time to schedule any testing if needed by our doctors.

Do you accept walk-in appointments?

Patients will need to schedule their appointments in advance by calling the office. Walk-in appointments are not permitted.

Do you offer language interpretation?

We offer sign language interpretation upon request. It is advised to bring somebody with you if your primary language is not English, as there might not always be a staff available to interpret.
